NTSB Narrative Summary Released at Completion of Accident

On June 6, 1994, approximately 1200 mountain daylight time, a Bell 204B helicopter, N109CH, was substantially damaged during an emergency landing near Alamogordo, New Mexico. The aircraft, flown by a commercial pilot, was on a public use fire fighting flight. There was no flight plan filed and visual meteorological conditions prevailed. The pilot, the sole occupant, was not injured. During the fire fighting mission, the pilot returned to a 7,500 foot MSL meadow for a water pick up. According to the pilot, he was hovering the aircraft over a water tank while filling an external bucket when he experienced a partial power loss. During the emergency descent the bucket and sling became entangled in the side frame of the portable tank. As a result of the entanglement, the pilot was forced to land adjacent to the tank and the tail boom contacted the tank on touchdown. Investigation revealed that the variable inlet guide vane control rod was improperly installed between the actuator and the fuel control.
