NTSB Narrative Summary Released at Completion of Accident

On June 25, 1999, a Beechcraft T-34B, Dominican Republic registration 1611 FAD, registered to and operated by Fuerza Aerea Dominicana crashed about 40 yards outside the air force base fence in San Isidro, Dominican Republic, while on a test flight following engine installation and structural work. The aircraft was destroyed, and both pilots were fatally injured. The overhauled engine installation and structural work had recently been performed, and the follow-on test flight remained in the traffic pattern returning for a landing, due to a fuel leak caused by improper fuel cap installation. During the subsequent flight, soon after takeoff at about 300 feet agl, the engine failed and the aircraft crashed. There was no postcrash fire. The Government of the Dominican Republic requested assistance from the NTSB and it was provided. The accident investigation is under the jurisdiction of the Government of the Dominican Republic.
