NTSB Narrative Summary Released at Completion of Accident

On December 6, 1999, about 0515 universal coordinate time (UTC), a Piper PA31-350 Chieftain, Republic of South African registration ZS-OJY, crashed shortly after takeoff from Rand Airport, Republic of South Africa. The airplane, operated by Flightline Charter Services CC, Republic of South Africa, was on a flight to carry computer company passengers to Namibia for training. The airplane was destroyed in the impact and post crash fire, and the pilot and all nine passengers on board were fatally injured. It is believed that the weather at the time of the accident was visual meteorological conditions, and the temperature about 58 degrees Fahrenheit. It is not yet know what type of flight plan was filed, in the flight conducted under the regulations of the Civil Aviation Authority (CAA), Republic of South Africa. After takeoff from runway 29 at Rand Airport (elevation 5,474 feet mean sea level), the pilot reported to air traffic control that there was a loss of power on an engine and he wanted to return to the airport for landing. According to witnesses, the airplane passed low over a nearby highway and then impacted in rocky terrain near a stream, about 2 nautical miles west of the runway. The airplane was manufactured by Piper in 1974, as serial no. 31-7405210. The Textron Lycoming engines were model TIO/LTIO-540-J2BD (Serial nos.: Left engine L-6729-61A, Right engine L-2037-68A). Total time on the engines is unknown. The left engine was shipped from Textron Lycoming, January 28, 1994; the right engine shipped, January 20, 1994. The pilot's flight times and experience are not yet known. The U.S. Accredited Representative is Thomas R. Conroy, Senior Air Safety Investigator, NTSB, AS-20, Washington, D.C. 20594, tel.
