NTSB Narrative Summary Released at Completion of Accident

On March 22, 2000, at 1430 eastern standard time, a Helio 700 airplane, Ecuadorian registration HCCAJ, was substantially damaged during a forced landing following a loss of engine power during initial takeoff climb near Miazal, Ecuador. The airplane was owned and operated by Mision Salesiana del Oriente, of Quito, Ecuador. The commercial pilot and the 5 passengers aboard the airplane sustained serious injuries. Visual meteorological conditions prevailed for the missionary support flight for which a company VFR flight plan was filed. The flight was originating at the time of the accident. The operator reported to the Ecuadorian authorities that the airplane departed a 2,000-foot airstrip with 5 passengers and 100 pounds of cargo. The 12,000 hour pilot reported that the engine lost power without warning while he performed the first power reduction during initial takeoff climb at approximately 700 feet agl. The pilot attempted to return to the airstrip, impacting trees and terrain about 1,500 meters short of the threshold of the runway. The investigation is under the jurisdiction and control of the Government of the Republic of Ecuador.
