UPON TAKEOFF, ACFT LIFTED OFF THE RWY AND SETTLED HARD BACK ONTO THE RWY. INSPECTION REVEALED ACFT WAS COVERED WITH 1/4 TO 1/2 INCH OF SNOW & ICE WHEN THE TAKEOFF WAS ATTEMPTED.
