NTSB Narrative Summary Released at Completion of Accident

On August 16, 2002, about 1355 local time (0055 universal coordinated time, UTC) a Hughes-269C, ZK-HFW, crashed while on an agricultural spraying mission near Dannevirke, North Island, New Zealand. The pilot was fatally injured and the helicopter was destroyed in the impact. The accident occurred in visual meteorological conditions and the helicopter was being operated under the authority of the Civil Aviation Authority, New Zealand. A witness reported observing the helicopter spraying above an incline and over a ridge, and then hearing a sound described as "engine screaming." The witness reported observing the helicopter at about 2 kilometers in a descent before it passed from view, followed by a sound of aircraft impact. The pilot, the only person on board, was found fatally injured. The investigator-in-charge, Civil Aviation Authority, New Zealand, continues in the investigation, which includes engineering and metallurgical examinations, including examination of the tail rotor adapter spline.
