NTSB Narrative Summary Released at Completion of Accident

On November 5, 2000, a Boeing 747-200 registerd as TJ-CAB, operated by Cameroon Airlines, providing scheduled service between Douala and Paris Charles de Gaulle after a landing roll of about 1700 meters on runway 9L suddenly deviated to the right of the runway center line and left the runway between taxiways Z6 and Z7. When the aircraft passed over a rainwater collector tank, the nose gear was torn off and damaged the fuselage at the level of the electronics bay. The aircraft came to a stop on the concrete structure of the tank. The aircraft was evacuated without injury to the 187 passengers. Three cabin crew were slightly injured. Thirteen other crew members were uninjured. An examination of the airplane determined that the thrust reversers on both left engines were in the retracted position and the right enginethrust reverers were in the extended position. The Bureau D'Enquetes Et D'Analysis ( BEA) of France investigated the accident and in its report number tj-b001105 determined that " the initial cause of the accident was the incomplete reduction of thrust on the left outer engine at the beginning of deceleration. This caused the de-activation of the automatic braking systems and the non-extension of the number 1 thrust reverser. The inadvertent selection of full thrust on this engine after landing created high thrust asymmetry leading to the runway excursion. The lack of coordination and of joint control by the crew members, perhaps aggravated by the presence of third parties in the cockpit, contributed to the development of this situation."
