NTSB Narrative Summary Released at Completion of Accident

On November 01, 2003, about 0830 central standard time, an Ayres Corporation S2R, Costa Rican registration TI-AYE, registered to and operated by Servicio Nacional de Helicopteros, collided with a house while fumigating crops at Finca la Victoria, San Julian de Zapote, Puerto Viejo de Sarapiqui, Costa Rica. Visual meteorological conditions prevailed at the time and no flight plan was filed. The airplane was substantially damaged and the commercial-rated pilot incurred minor injuries. Four people on the ground received minor injuries and one person was seriously injured. The flight originated from Sarapiqui, at about 0800. Information received by the NTSB indicated the aircraft was on the fifth application run when the pilot for unknown reasons could not continue the operation and elected to head toward an open field. In the process the pilot encountered trees, he lowered the flaps and added power in an attempt to clear the trees. The pilot selected to dump the remaining chemical overboard when the applied settings did not aid in flying over the trees. The airplane stuck the trees and collided with a house, which was occupied at the time. The investigation is under the jurisdiction of the government of Costa Rica.
