NTSB Narrative Summary Released at Completion of Accident
On November 13, 2003, approximately 1830 universal coordinated time (UTC), a Bell 206L4 helicopter, Brazilian registration PT-YEE, was destroyed upon collision with terrain while maneuvering near Urucu, Province of Amazonas, in the Republic of Brazil. The commercial pilot and one passenger were fatally injured, and 3 other passengers sustained serious injuries. The helicopter, serial number 52173, was owned and operated by Helisur in support of Petrobraz. Visual meteorological conditions prevailed and a company visual flight rules (VFR) flight plan was filed to the local gas exploration support flight. The flight originated from a company base camp near Urucu. The operator was alerted of the accident after the pilot of the helicopter failed to make a required 15-minute report.
The accident investigation is under the jurisdiction and control of the Division of Investigation and Prevention of Aeronautical Accidents (DIPAA) of the Department of Civil Aviation of Brazil. Additional information may be obtained from:
