NTSB Narrative Summary Released at Completion of Accident

On November 13, 2003, approximately 1830 universal coordinated time (UTC), a Bell 206L4 helicopter, Brazilian registration PT-YEE, was destroyed upon collision with terrain while maneuvering near Urucu, Province of Amazonas, in the Republic of Brazil. The commercial pilot and one passenger were fatally injured, and 3 other passengers sustained serious injuries. The helicopter, serial number 52173, was owned and operated by Helisur in support of Petrobraz. Visual meteorological conditions prevailed and a company visual flight rules (VFR) flight plan was filed to the local gas exploration support flight. The flight originated from a company base camp near Urucu. The operator was alerted of the accident after the pilot of the helicopter failed to make a required 15-minute report. The accident investigation is under the jurisdiction and control of the Division of Investigation and Prevention of Aeronautical Accidents (DIPAA) of the Department of Civil Aviation of Brazil.
