NTSB Narrative Summary Released at Completion of Accident

On February 28, 2004, approximately 1730 local time, a Sikorsky SK-64 helicopter, N158AC, sustained substantial damage following impact with trees during descent near Bintutu, Malaysia. The helicopter was registered to and operated by Erickson Air-Crane Inc., of Central Point, Oregon. The pilot-in-command, and the pilot acting as second-in-command were not injured. Visual meteorological conditions prevailed and no flight plan was filed for the local flight. It was reported that during a long-line logging operation, the helicopter was descending with a 275-foot long-line with a hydraulic grapple preparing to secure a log. During the descent the main rotor blades came in contact with a tree, resulting in substantial damage to the rotor blades. The pilot was able to fly the helicopter back to the landing zone and land without further incident. The investigation is under the jurisdiction of the government of Malaysia.
