NTSB Narrative Summary Released at Completion of Accident

On June 25, 2001, an AeroMexico MD-82 experienced an uncontained low pressure turbine event in the No. 2 engine, a P&W JT8D-219, during approach to Guadalajara, Mexico. While performing a go-around, both engines were advanced to full power, and approximately 5 seconds later, the No. 2 engine failed. Examination of the No. 2 engine revealed a 14 x 5-inch holes in the upper case of the fan and combustion chamber assembly. All the rear turbine case-to-exhaust case bolts from the 8:00 to 4:00 o'clock positions were fractured and the rear turbine case and exhaust case mating flanges were pushed outward and separated creating a gap. Neither of the two engine cases were penetrated. All the low pressure turbine (LPT) 4th-stage blades were fractured transversely across the airfoil just above the platform with all the blade roots still attached to the disk. Metallurgical examination of the rear turbine case-to-exhaust case bolts revealed that they all fractured due to overload and that one of the fractured LPT 4th-stage blades exhibited arrest marks typical of fatigue. The fatigue origin was obliterated by mechanical damage so that no conclusive determination of whether any pre-existing anomalies existed or to obtain an exact cycle count. The investigation is under the jurisdiction of the government of Mexico.
