NTSB Narrative Summary Released at Completion of Accident

On July 25, 2001, at 1405 UTC, Sabena flight 581, an Airbus Industrie A-330 airplane, registered in Belgium as OO-SFM, lost power in the No. 2 engine, a General Electric Aircraft Engines CG6-80E1A2, while climbing through FL230. The pilot reported that the engine surged and the airplane yawed and vibrated. The pilots shutdown the engine and returned to Brussels, Belgium for an uneventful single engine landing. The examination of the engine after landing revealed that there were no holes in the cowling, but that there was an approximately 300 degree circumferential hole in the turbine case in the plane of the low pressure turbine stage 5 rotor. The airplane was operating as a regularly scheduled passenger flight from Brussels to Montreal, Canada. There were no reported injuries to the 2 pilots, 9 flight attendants, and 254 passengers. This incident is under the jurisdiction of the Government of Belgium.
