NTSB Narrative Summary Released at Completion of Accident

On August 13, 2004, at 1300 Pacific daylight time, a Robinson R22 Beta helicopter, Canadian registry C-FHGH, descended into a lake during the intial climb after takeoff near Campbell River (McIvor Lake), British Columbia, Canada. The pilot, the sole occupant, sustained fatal injuries; the helicopter was destroyed. Visual meteorological conditions prevailed for the local area flight. The helicopter was departing a helipad when the accident occurred. The accident is under the jurisdiction of and is being investigated by the Transportation Safety Board of Canada.
