NTSB Narrative Summary Released at Completion of Accident

On October 31, 2004, approximately 1040 eastern standard time, a Bolkow BO-105-LS twin-engine helicopter, Dominican Republic registration HI-6875P, was substantially damaged during a forced landing following a loss of engine power during cruise flight near Puerto Plata, Dominican Republic, its intended destination. The pilot and one passenger were seriously injured, and the other two passengers sustained minor injuries. The helicopter, serial number 2035, was owned and operated by a private individual. Visual meteorological conditions prevailed for the domestic cross-country flight that originated from Santo Domingo, Dominican Republic, at 1010. Local aeronautical authorities reported that the remains of the helicopter were being recovered to a secured location at the Herrera Airport for further examination and testing. Fuel was found in the fuel cells and within the external fuel filters for both engines. The accident investigation is under the control and jurisdiction of the Government of the Dominican Republic.
