NTSB Narrative Summary Released at Completion of Accident

On April 16, 2005 at 1055 local time, a Sikorsky S61 helicopter, a commercial flight, Malaysian registry 9M-PCM, experienced a loss of control during takeoff and force landed near Kerteh Airport, Terengganu, Malaysia. Visual meteorological conditions existed. The two flight crewmembers , and six of the seven passengers were not injured. One passenger received minor injuries during the evacuation of the helicopter. The helicopter was substantially damaged. There was no fire. The departure point was Kerteh Airport and the destination was an off shore oil rig. During takeoff climb, at an altitude of 200 feet, the flight crew experienced loss of response from the yaw control. Despite the flight crew attempts to regain control, the helicopter continued to yaw left and lose altitude. The flight crew subsequently made a forced landing about a mile south of the airport. The investigation is being conducted by Idros Bin Abd Rahman, Chief Inspector of Air Accidents, Ministry of Transport, Malaysia.
