NTSB Narrative Summary Released at Completion of Accident
On May 24, 2006, about 0055 eastern daylight time, a Israel Aircraft Industries 1124, N475AT, registered to Air Ambulance By Air Trek, Inc., operated by Air Trek, Inc., ran off the side of the runway during an emergency landing at Exuma International Airport, Georgetown, Bahamas. Visual meteorological conditions prevailed at the time and an instrument flight rules flight plan was filed for the 14 CFR Part 135 Air Medical Transport Flight from Luis Munoz Marin International Airport, San Juan, Puerto Rico, to Norfolk International Airport, Norfolk, Virginia. The airplane was substantially damaged and the airline transport-rated pilot and co-pilot sustained minor injuries. The three medical members sustained minor injuries, and the patient, and 2 passengers were not injured. The flight originated about 2315, from Luis Munoz Marin International Airport.
The pilot stated that while en route at an unspecified location "...we received a Left GEN OFF annunciator light and I had the First Officer accomplish the GEN OFF [Quick Reference Handbook]." The Quick Reference Handbook (QRH) was completed and the flight continued with the right generator functioning normally. The decision was made to divert to Charlotte County Airport, Punta Gorda, Florida, and the co-pilot notified air traffic control (ATC) of this decision. The flight continued and approximately 1 hour 40 minutes into the flight he noticed several annunciations, and messages that both GPS receivers were not receiving input from the #2 electrical bus. The right generator was checked and it appeared to be functioning normally, but when he later checked the battery gauge it was reading 11-14 volts. He declared an emergency with ATC and advised the controller of their situation, and requested a vector to the nearest airport. He also was aware that the GPS indicated that Santiago and Exuma were the closest airports, and because of his knowledge of the Exuma airport, requested to be vectored to that airport. The flight was vectored to the Exuma International Airport and while fully configured for landing while flying at 1,500 feet, the airplane, "...became dark." No lights were on at the airport and he spotted what he thought was the runway. After touchdown, he applied full brakes and the thrust reversers but they did not deploy. At some point during the landing roll the right main landing gear tire "blew" and the airplane departed the right side of the runway which he was unsuccessful in correcting. After coming to rest he attempted to secure both engines but the left engine continued to operate. The airplane was evacuated and they remained near the airplane until the authorities arrived.
