NTSB Narrative Summary Released at Completion of Accident

While climbing out from Sapporo Japan (New Chitose Airport) enroute to Anchorage, Alaska, a Kalitta Air 747-200B, powered by Pratt & Whitney JT9D-7Q engines experienced a fire in the No. 3 engine nacelle. The flight crew reported that the No. 3 engine fire warning light illuminated just after takeoff. The pilot declared an emergency, shutdown the No. 3 engine, and discharged a fire bottle. While returning to Sapporo, the flight crew jettisoned excessive fuel for landing. An eventful three engine landing was made. Examination of the aircraft revealed damage to the right hand cowling of the No. 3 engine and heavy sooting and thermal damage to the right side of the No. 3 engine from about the high compressor case aft to the high pressure turbine case. A leak check of the engine was performed on site and revealed a fuel leak coming from approximately the 1:00 o'clock position in the vicinity of the No. 2 and No. 3 fuel nozzles. The investigation is under the jurisdiction of the government of Japanese ARAIC.
