NTSB Narrative Summary Released at Completion of Accident

On February 20, 2007, a Bell Helicopter 212, Brazilian registration, operated by Omni Helicopters, Sao Paulo, Brazil, landed in a field after experiencing severe vibrations during a flight from an oil platform originating in the Atlantic Ocean. During the post landing inspection, one of the two main rotor blade grips was found cracked. The blade grip had been inspected about 109 flight hours before in accordance with AD 2003-01-04, which specified an inspection interval of 150 flight hours. The blade grip was sent to the manufacturers laboratory in Ft Worth, Texas for examination. Along with the cracked part, several inspection methods were reviewed. The procedures review showed that the blade grip had not been inspected in accordance with AD 2003-01-04, nor the manufacturers approved procedures. The Omni Helicopter company employee was given a class on the correct procedure to follow. The investigation is under the jurisdiction of the government of Brazil.
