NTSB Narrative Summary Released at Completion of Accident

On February 13, 2003, at about 1255 local time, a Japan Asia Airlines (JAA) Boeing B747-200B, registration JA8129, powered by four P&W JT9D-7A engines, was in cruise flight at 28,000 feet when the flight crew heard a bang and felt airframe vibrations. The No. 2 engine was shut down in-flight and the airplane diverted to Okinawa, Japan. There were no reported injuries. Ground inspection showed that the Boeing inlet cowl was penetrated, and one fan blade had fractured and seperated approximately 20 cm. outboard of the platform. There was no reported aircraft damage. The flight was scheduled from Narita, Japan to Taipei, Taiwan. The investigation is under the juristiction of the government of Japan.
