NTSB Narrative Summary Released at Completion of Accident

On February 1, 2006 at 19:56 UTC, a CargoLux Airlines Boeing 747-400 (registration LX-GCV), equipped with GE CF6-80C2 engines, sustained a partial fan blade release and un-containment on the number 4 engine shortly after touchdown and during the thrust reversal phase while landing at Istanbul, Turkey airport (IST). The ACARS data indicated a sudden increase in fan vibration on the number 4 engine at approximately 35.6 percent N1. The following damage was reported during inspection of the number 4 engine: - One fan blade was missing the blade tip at the location just below the mid-span damper. The other fan blades had severe leading edge tears and missing material. - The fan case was battered around 360 degrees on the fan plane as well as forward of the fan plane. The inner skin was torn and missing patches in several locations, exposing the honeycomb core. - The fan case kevlar containment ring was penetrated allowing the escape of a fan part at the 8 o'clock location. - The outer guide vanes were damaged. The engine was removed from the aircraft and sent to KLM for repair. The fan case and fan blades were sent to GE for evaluation. The GE prepared metallurgical report stated that no anomolies were found in the fan blade material and further that a 'soft body' deformation was found in one of the blades, indicating possible foreign object ingestion. The Turkish Authorities chose not investigate this incident.
