NTSB Narrative Summary Released at Completion of Accident

On February 3, 2010,at 1041 UTC, a Thai Airways Boeing 747-400 experienced an oil filter message, high vibration, and fire warning from the No. 1 engine, a General Electric CF6-80CB1F turbofan engine, shortly after takeoff from the Suvarnabhumi International Airport (BKK), Bangkok, Thailand. The flightcrew shutdown the No. 1 engine, discharged both fire extinguishing bottles, and started a return back to the BKK airport. A successful three engine landing was performed with no reported injuries. The flight was scheduled from BKK to Chiang Mai International Airport (CNX), Chiang Mai, Thailand. Examination of the airplane revealed several burn through holes on the outboard fan cowl and thrust reverser translating sleeve. Thermal damage was noted to the engine’s compressor rear frame, to the Integrated Drive Generator (IDG), and the angle gearbox. The investigation is under the jurisdiction of the government of Thailand.
