NTSB Narrative Summary Released at Completion of Accident

An Airbus A319-131, registration number G-EUOE, during departure from London-Heathrow International Airport, had the engine cowls from both International Aero Engines V2500 engines separate and fall on to the runway. The pilots declared a PAN and reported that they would return to Heathrow. During the approach to Heathrow, a fire occurred in the right engine. The pilots declared an emergency, shut down the right engine, and discharged both fire bottles. After the airplane landed and was stopped on the runway, the fire brigade extinguished a small fire in the right engine. The passengers and crew evacuated the airplane via the emergency slides on the left side of the airplane. The Air Accidents Investigation Branch (AAIB) of the United Kingdom is investigating the accident. As the state of manufacture of the engines, the NTSB has designated a U.S. Accredited Representative under the provisions of Annex 13 to the Convention on Civil Aviation to assist the AAIB with their investigation. All inquiries concerning this accident should be directed to the AAIB of the United Kingdom: Air Accidents Investigation Branch Farnborough House Berkshire Copse Road Aldershot, Hampshire GU11 2HH, United Kingdom Tel.: +44 (0)1252 510300 Telex: 858119 ACCINV G Fax: +44 (0)1252 376999
