NTSB Narrative Summary Released at Completion of Accident

On November 5, 2013, about 1912 central standard time, a Piper PA-32R-301, N408DM, registered to RFG LLC and operated by Wisconsin Aviation, collided with trees and the terrain during an instrument approach to the Kirksville Regional Airport (KIRK), Kirksville, Missouri. The flight instructor and private pilot were fatally injured. The airplane was destroyed. The business/instructional flight was being operated under 14 Code of Federal Regulations Part 91. Instrument meteorological conditions existed and an instrument flight plan was filed. The flight originated from the Centennial Airport (KAPA), Denver, Colorado, about 1543. According to the operator of the airplane, the flight instructor was on a "mentor" flight with the private pilot. The private pilot had flown to Denver for a business trip and was receiving instrument flight instruction during the trip. A member of the private pilot's family stated that the airplane was returning to Wisconsin and was stopping at KIRK for fuel when the accident occurred. The airplane was cleared for the global positioning system (GPS) 18 approach and had been issued a frequency change to the common traffic advisory frequency at KIRK. Personnel at KIRK stated the pilot of N408DM checked in on the frequency, and was given the wind and altimeter information. The pilot did not reply, but had keyed the radio several times to turn on the pilot controlled airport lighting. When the airplane did not land, personnel from the fixed base operator (FBO) at KIRK attempted to contact the airplane to no avail. FBO personnel then received a call from the Kansas City air traffic control center inquiring about the airplane. Local authorities began searching for the airplane which was subsequently located about two hours later in a wooded area four miles north-northeast of the airport.
