NTSB Narrative Summary Released at Completion of Accident
On June 3, 2014, about 1418 Pacific daylight time, an experimental amateur-built Phillips BD 4, N11DV, collided with terrain during approach to landing at Dayton Valley Airpark, Dayton, Nevada. The owner was operating the airplane under the provisions of 14 Code of Federal Regulations (CFR) Part 91. The airline transport pilot sustained fatal injuries. The airplane sustained substantial damage during the accident sequence. The local personal flight departed at an undetermined time. Visual (VMC) meteorological conditions prevailed, and no flight plan had been filed.
Witnesses observed the airplane as it approached for landing. As the airplane was turning base to final approach, it entered a steep nose-down spiral, and continued into the ground.
