NTSB Narrative Summary Released at Completion of Accident

On May 18, 2014 about 1015 central daylight time, a Texas Helicopter Corp OH 13H/M74A agricultural helicopter, N89849, registered to the pilot, sustained substantial damage following a collision with trees and a building near Little Falls, Minnesota. The commercial pilot sustained serious injuries during the accident and later succumbed after hospitalization. The aerial application flight was conducted under the provisions of 14 Code of Federal Regulations Part 137. No flight plane was filed and visual meteorological conditions prevailed in the vicinity. The flight originated at 0945 from a private agricultural staging area. Evidence at the accident site revealed that the tail rotor of the helicopter contacted trees prior to impacting the rooftop of a building. Examination of the helicopter airframe, flight controls, and engine did not reveal any mechanical anomalies.
