NTSB Narrative Summary Released at Completion of Accident

On August 26, 2014, about 1830 Pacific daylight time, an amateur-built Thomas C. Sikes, Loehle 5151 Mustang, N73144, was destroyed following a loss of control during initial climb about 0.14 miles east of the approach end of runway 34, Chehalis Airport-Centralia Airport (CLS), Chehalis, Washington. The pilot, who was the registered owner of the airplane and the sole occupant, sustained fatal injuries. Visual meteorological conditions prevailed and the time of the accident, and no flight plan was filed for the local flight. The airplane, which was originating at the time of the accident, was being operated in accordance with 14 Code of Federal Regulations Part 91. A witness, who is a professional pilot and who was positioned on the airport's south perimeter road and behind the accident airplane, reported to the National Transportation Safety Board investigator-in-charge, that the pilot started his takeoff roll well back from the approach end of the runway on the grass. As the airplane's speed increased on the takeoff roll he observed it bounce due to the uneven terrain. The witness further reported that the airplane subsequently made it onto the runway's surface and became airborne, but the initial climb was not stabilized; the airplane's wings were rocking from side to side, and it's nose was pitching up and down. The witness stated that he then observed the airplane make a right turn of about 180 degrees, and still climbing in an unstable attitude. The airplane the went into a right bank and steep nose down attitude from an altitude of about 500 feet above ground level prior to impacting terrain to the east of his location. The airplane impacted terrain in an fenced in yard of a local automotive repair facility, about 750 feet east of the where the takeoff run began; the location was also only about 165 feet east of a major interstate highway. Impact forces and the subsequent fire that ensued consumed the airplane, which was almost entirely of wood construction.
